Yea, I think so.
You just change the cartridges to super T ones. I think it's a 100 bucks.

Yes it's possible, you have to order the Super-T HSCV kit. (around 130$)
It's only the right side fork leg that 'needs' to be upgraded to have a 2004 Super-T.

tis possible, seeing as the bodies of the super t and jrt are the same. All youd have to do is find the internals and a decal set. maybe go to the lbs and ask if they can get supert internals and decals from marzocchi. oh, don't forget the top caps too, since they are diffrent. after that, it's pritty easy. drain the oil, pull out the internals, loosen both top and bottom ends, swap the springs and then do the same thing in reverse order, but i'm sure if your asking, you know a little bit about it.
as far as i recall, the 03/04 supert came with air assist or something like that, but it's said that 03 was the best year for both the super and jrT

Any LBS that carries Marzocchi should be able to order it for you.
The 03 Jr-t (with 32mm stanchions) is the same chassis as the 2004.

Actually you don't need new top caps, just have to perform a little surgery on them. I don't think there ever was a best year for the SuperT. 03 had poor tire clearance and no adjustable lower crown, 04 had only 1 HSCV cart. (some have said that the SSV cart. was drilled out on these models?) and also stupid QR20. An 05 Junior upgraded with HSCV would be a great fork, no QR20, good tire clearance, good damping, and adjustable lower crown!
The best year super was the 03, as long as you can get over the tire clearance. It had 2 HSCV carts which made a huge difference. If you're running one SSV cart and one HSCV cart then run really light oil in the SSV side (2.5 wt ish) so you won't feel it spiking.
If you can afford it, buy 2 HSCV carts and run them both. You DO need new top caps, the super-T carts have a rod that threads into the top cap and the adjustment rod pokes through the cap.

No, you do not need new top caps. All you have to do is remove the preload knob, remove the c-clip, and take out the threaded part.

no, you're wrong the HSCV has the rebound sticking out the top of the cap. the SSV(F) has the rebound internal or on the lowers

How do you guys perform the swap? Do you just swap cartridges or do you make any other modifications? (drilling, different oil weight, etc...)

Yeah, and once you remove the threaded piece from inside the top cap, the top of the cartridge threads into the topcap and the rebound adjustment screw pokes through.

What would it be like with dual HSCV carts in it?

the 02 rocked. but it had the 2 piece lowers that occasionally seperated

It would feel just like an 02 Monster T (dual HSCV), but not as stiff. The damping feels more plush and controlled, and the spiking problem is eliminated.
